  • Overview

    Provides ongoing strategic direction and management of the Hospitalist and Post Acute Care Program at AltaMed. The program will include all aspects of inpatient services including admissions or diversions from the emergency department, skilled nursing facility (SNF) transfers and admissions, inpatient and SNF concurrent review and utilization management, discharge planning, and transitions of care collaboration, for both an employed and contracted hospitalist program.

     

    Responsibilities

    1. Manages and oversees hospitalists and SNFists who provide hospitalist services to AltaMed members admitted to designated hospitals and SNFs throughout a network of hospitals and SNFs across the greater Los Angeles area.
    2. Provides leadership and management for hospitalists which will include daily telephonic and/or in-person rounding with hospitalists and designated members of the inpatient medical management team. Will provide guidance and clinical subject matter expertise on concurrent review of both hospital and SNF cases for AltaMed members.
    3. Develops coverage schedule for hospitalists, both employed and contracted, to ensure that both inpatient and post-acute care coverage is provided 24 hours a day, 365 days a year. Provides schedule to key stakeholders including the inpatient medical management department.
    4. Facilitates discussions between hospitalists and the inpatient medical management team with regard to concurrent review as well as care transitions.
    5. Facilitates the clinical decision making and transfer of members, where appropriate, from out of network hospitals to in-network hospitals.
    6. Attends and participates in regularly scheduled Joint Operating Meetings between AltaMed and key hospitals throughout the Los Angeles area.
    7. Oversees and provides quality improvement recommendations around documentation by hospitalists for both inpatient and SNF care. This will include review of admission and /or consultation notes, progress notes, discharge summaries and where applicable, hand-off communication between hospitalists. Ensures that documentation is in accordance with rules and regulations as determined by the appropriate governing body or guideline (ie. DHCS, Joint Commission, Health and Safety codes, etc.)
    8. Provides ongoing development of current ER diversion program which aims to provide members with the most appropriate level of care as recommended by evidence based guidelines.
    9. Provides education and guidance to hospitalists and members of the inpatient medical management team around complex cases involving end of life and palliative care treatments. The role will require having significant experience with tools such as POLST and Advance Directives.
    10. Is accountable for identifying and reporting key issues around quality of care, including hospital acquired conditions, never events, etc.
    11. Sets communication standards for hospitalists in their communication with community PCPs with regard to discharges and admissions.
    12. Provides feedback and monitors key performance indicators for the AltaMed Hospitalist and Post-Acute Care program. This includes reviewing admission and re-admission rates, lengths of stay, bed-days, and outlier cases for both inpatient and SNF
    13. Participates in the setting of goals for inpatient and SNF with regard to bed day and admit rates, as relevant to annual budget establishment

     

    Qualifications

    1. Current unrestricted California license. Must be board certified in Internal Medicine and/or subspecialty of Internal Medicine
    2. Minimum of 5 years of clinical hospitalist experience, of which at least 2-3 years must be in the management of inpatients under managed care payer (HMO).
    3. 1-3 years of leadership experience in leading an inpatient program highly preferred.
    4. Current DEA certificate
