  • Overview

    The Director, Medical High Risk Visits has overall responsibility for the clinical patient care for the High Risk Clinic/Home visit Program. He/she addresses clinical, programmatic, medical, reporting and administrative issues with the program in collaboration with the Clinic Administrator and Nurse Administrator at clinic sites, and with Regional Medical Directors, Case Management leadership as necessary.

     

    Responsibilities

    1. Supervise all medical services offered by the program and provides primary care to high risk populations.
    2. Ensure program staff and delivery of care emphasize core concept of high risk patient clinic care including but not limited to chronic disease management of frail, elderly, and populations with many social determinants of health; palliative care; end of life care; acute high level urgent care services; advanced life planning, family conference; realistic goal setting; patient self_management skills; emergent SNF placement.
    3. Reviews physician and mid-level professional staff performance on quality, patient experience and appropriate resource use at the site no less than monthly, and performs individual provider evaluations no less than annually.
    4. Orient/trains all new physician and mid-level providers, and oversee coverage and call schedule in coordination with VP Medical Management.
    5. Supervises the work of employees including hiring/terminating, as well as, responsibility for planning and scheduling work, discipline/performance management and salary recommendations.
    6. Oversees the management of participant’s medical situations, and oversees the participant’s use of medical specialists and inpatient care.
    7. Assist in data collection analysis and preparing reports regarding clinical service delivery.
    8. Responsible for provider coverage in clinical emergencies, and assists in the development of Program clinical policies and protocols.
    9. Represent AltaMed to external providers and organizations regarding patient management and clinical issues as appropriate.
    10. Assists in managing and monitoring the cost of hospitalization, specialty and ancillary services and identifies, implements and measures areas for improvement.
    11. Work with VP Medical Management to carry out coordinated communication process between hospitalist and Physician/Clinic Staff.
    12. Work with VP Medical Management to carry out collaborative process for working with facility discharge planning staff and contracted facilities.
    13. Ensure that chart audits are being conducted, identify problems and provide guidance for resolution.
    14. Work to develop, implement and monitor clinical guidelines that are appropriate for the assigned population in coordination with Medical Management department.
    15. Work with High Risk clinic/Home providers to ensure that pharmaceutical care meets State requirements, and professionally recognized standards of practice.
    16. Ensure that effective drug reviews and pharmacy policies are implemented.
    17. Ensure that peer review is performed regularly for hospital based providers and clinic based ancillaries.
    18. Assist in the development and implementation of the Corrective Action Plans for periodic audits by outside agencies.
    19. Performs other related duties as assigned.

    Qualifications

    1. Physician with an active unrestrictive CA license to practice medicine by the Medical Board of California and DEA License required.
    2. Prior experience in a leadership or management role strongly preferred.

    3. Prior experience with chronic disease care, palliative care/hospice strongly preferred.

    4. Previous experience with computer-based systems required; Electronic Medical records experience preferred.

    5. Education, training and experience necessary to meet the underwritten requirements for inclusion under AltaMed’s malpractice insurance coverage.

    6. Current CPR and First Aid certificate required.

    7. Bilingual: English/Spanish/Mandarin/Cantonese preferred depending on location.
